Dyslipidemia, defined as elevated levels of total cholesterol, low-density lipoprotein (LDL) cholesterol, or triglycerides, or low levels of high-density lipoprotein (HDL) cholesterol, is a major risk factor for CVD [4]. As with other CVD risk factors, genetic risk plays a role in the development of dyslipidemia, such as in familial hypercholesterolemia; however, the majority of cases are due to unhealthy lifestyle behaviors [5,6].

Dyslipidemia refers to elevated total cholesterol, low-density lipoprotein cholesterol (LDL-C), or triglycerides, or low levels of high-density lipoprotein cholesterol (HDL-C) [3]. While this condition can be due to genetic factors, it is usually associated with unhealthy lifestyle behaviors such as poor diet and physical inactivity.

The Cholesterol Care Plan is a 12-week care plan for self-management of cholesterol focusing on lifestyle and medication compliance, with weekly reminders and educational videos designed using evidence-based guidelines such as AHA’s CVD risk, cholesterol, and lifestyle guidelines [13,27,28]. Using 7 health factors and lifestyle behaviors that support heart health, it is based on the AHA’s “ideal cardiovascular health.”

Sustained Use of Patient Portal Features and Improvements in Diabetes Physiological Measures
We used the American Diabetes Association’s guidelines to define cutoffs for glycemic, cholesterol, and blood pressure control [25]. We defined uncontrolled blood glucose at baseline (April 2009-March 2010) if the patient’s average hemoglobin A1c (Hb A1c) during that period was greater than or equal to 7.0% (53 mmol/mol) and uncontrolled LDL cholesterol at baseline if the patient’s average cholesterol reading during that same period was greater than or equal to 100 mg/d L (2.586 mmol/L).
