As part of the development of MT4 C, Duggleby et al’s [22] adaptation of Meleis’ transition theory, involving core concepts of acknowledging the situation, connecting, and redefining normal, was mapped to the specific components of MT4 C (for more information see Duggleby et al) [22]. The Web developers for this intervention were ATMIST [25].

A Web-Based Intervention to Help Caregivers of Older Adults With Dementia and Multiple Chronic Conditions: Qualitative Study

The Transition Toolkit developed by Duggleby, Swindle, and Peacock [25], was based on transitions theory [26]. The original paper-based Transition Toolkit was assessed in a pilot study and found to be feasible, easy to use, acceptable for use, and to have the potential to support caregivers through transitions [25]. On the basis of the pilot study, it was converted into a Web-based format: MT4 C.
