Health literacy was approximated using the Short Form Functional Health Literacy Assessment (S-TOFHLA) [23]. Level of patient engagement in their health and health care was measured using the Patient Activation Measure (PAM), which is a validated, 13-item instrument [24]. Subjects were asked to self-report their health status and presence of diabetes, hypertension, heart disease, arthritis, depression, tobacco use, asthma, or chronic pulmonary disease.
