For the development of a questionnaire guide (see Multimedia Appendix 2), we followed established recommendations [20] and used a team-based approach—M Hoffmann, sociologist; MW, implementation science; M Hartmann, psychotherapist; and MWH, internal medicine specialist—advancing to specific open-ended, jargon-free questions from the initial formulation of key questions in a logical sequence.
