Mohr et al [33] noted that when digital interventions that are shown to be efficacious in laboratory-based studies move to real-world settings, individuals do not engage with the tools and implementation often fails. Fourth, the complex issue of the ordering of interventions needs to be considered in population-based models. We, and others, have proposed stepped-care models as a cost-effective approach and also as a way to identify subpopulations that may need alternative approaches [14].
