The time-varying information on individual diagnoses; information on sex (male or female), age, educational attainment level (none, short, medium, long, missing, and missing before 1920); and calendar time were included as explanatory variables in the analysis (refer to tables 1/2 in the study by Holm et al [28]). We used an extended Cox model to estimate the effect of these explanatory variables on mortality, allowing for the inclusion of time-varying covariates.
