    <p>In the originally published article, affiliation 1 was not applied to author Mariana Pinto da Costa, and the affiliation <italic>“Centre for Mental Health, University Health Network – Toronto General, Toronto, ON, Canada”</italic> was not added for author Gerry Craigen.</p>
